Novel coronavirus infection - update  
The Ministry of Social Affairs and Health in France has informed WHO of an additional laboratory-confirmed case with infection of the novel coronavirus (NCoV).

This patient was identified as part of the epidemiological investigation initiated by the French authorities, following laboratory confirmation of the first case on 7 May 2013. The patient shared a hospital room in Valenciennes with the first laboratory-confirmed patient from 27 to 29 April 2013. The patient is currently hospitalized and isolated in an infectious disease hospital.

Among 120 persons identified as contacts of the first laboratory-confirmed case in France, laboratory tests were conducted by Pasteur Institute in Paris on five suspected cases, of which four tested negative, one (mentioned above) tested positive.

In Saudi Arabia, an investigation is ongoing into an outbreak in a health care facility, where 15 patients, including seven deaths have been confirmed.

From September 2012 to date, WHO has been informed of a global total of 34 laboratory confirmed cases of human infection with nCoV, including 18 deaths.

Based on the current situation and available information, WHO encourages all Member States to continue their surveillance for severe acute respiratory infections (SARI) and to carefully review any unusual patterns.

Health care providers are advised to be vigilant among recent travelers returning from areas affected by the virus who develop severe SARI. Specimens from patients’ lower respiratory tracts should be obtained for diagnosis where possible. Clinicians are reminded that nCoV infection should be considered even with atypical signs and symptoms in patients who are significantly immune compromised.

All Member States are reminded to promptly assess and notify WHO of any new case of infection with nCoV, along with information about potential exposures that may have resulted in infection and a description of the clinical course.

WHO does not advise special screening at points of entry with regard to this event nor does it currently recommend the application of any travel or trade restrictions.

WHO continues to closely monitor the situation.

Since the beginning of May 2013 to date, a total of 21 patients, including nine deaths, have been reported from the outbreak primarily linked to a health care facility in the Eastern part of Saudi Arabia. The government is conducting ongoing investigation into the outbreak.

From September 2012 to date, WHO has been informed of a global total of 40 laboratory confirmed cases of human infection with nCoV, including 20 deaths from 6 countries (France, Germany, Jordan, Qatar, Saudi Arabia, United Kingdom).

Minister Edith Schippers van Volksgezondheid, Welzijn en Sport heeft op basis van deskundigenadviezen besloten uit voorzorg een meldplicht in te stellen voor het nieuwe type coronavirus MERS-CoV (Middle East Respiratory Syndrome). De meldplicht heeft tot doel een eventuele patiënt zo snel mogelijk te ontdekken en het voorkómen van verdere verspreiding van de ziekte.
Geen concrete dreiging

Er is op dit moment geen sprake van concrete dreiging in Nederland. Het gaat om een voorzorgsmaatregel omdat niet is uit te sluiten dat een reiziger naar het Midden-Oosten, waar de ziekte voorkomt, deze meeneemt naar Nederland. GGD’en, huisartsen en ziekenhuizen zijn alert en weten wat ze moeten doen als zo’n geval zich zou voordoen. In geval van een daadwerkelijke uitbraak voert de minister de landelijke regie op de bestrijding en communicatie.

Het Middle East respiratory syndrome coronavirus (MERS-CoV) heeft nog weinig ‘pandemisch potentieel’. De ziekte lijkt in dat opzicht een stuk minder gevaarlijk dan SARS (severe acute respiratory syndrome). Onderzoek van Romulus Breban e.a. van het Parijse Institut Pasteur gepubliceerd in The Lancet wijst dat uit. De onderzoekers analyseerden gegevens van 55 van de inmiddels 64 bevestigde gevallen. WHO is convening an Emergency Committee under the International Health Regulations (IHR) for Middle East respiratory syndrome coronavirus (MERS-CoV). The Emergency Committee will meet on 9 and 11 July.

The Emergency Committee is made up of international experts to provide technical advice to the WHO Director-General in the context of a “public health emergency of international concern” (PHEIC). Depending on the circumstances, the Emergency Committee may advise on whether or not a PHEIC is occurring . If the Director-General determines that the event constitutes a PHEIC, the Emergency Committee will then provide advice on appropriate Temporary Recommendations of health measures to be implemented by States Parties. As a PHEIC proceeds, the Emergency Committee continues to provide advice to the Director-General on termination of the PHEIC, and the modification and termination of Temporary Recommendations. All decisions on these issues are taken by the Director-General.

5 July 2013 - The Ministry of Health (MoH) in Saudi Arabia has announced two additional laboratory-confirmed cases and two deaths in previously confirmed cases of Middle East respiratory syndrome coronavirus (MERS-CoV) infection in Saudi Arabia.

The new cases are a 69 year-old male and a 66 year-old male from Riyadh. Both were admitted to hospital on the 28 June 2013 and are currently in critical condition in an intensive care unit.

In addition, the two deaths in previously confirmed cases are a 63 year-old female from Riyadh and a 75 year-old male from Al Ahsa.

Globally, from September 2012 to date, WHO has been informed of a total of 79 laboratory-confirmed cases of infection with MERS-CoV, including 42 deaths.

Based on the current situation and available information, WHO encourages all Member States to continue their surveillance for severe acute respiratory infections (SARI) and to carefully review any unusual patterns.

Both the cases in Saudi Arabia have mild symptoms and are not hospitalized. They are from Asir region. The first case is a 26-year-old man who is a close contact with a previously laboratory-confirmed case and the second case is a 42-year-old woman who is a health care worker.

In the UAE, the four cases are health care workers from two hospitals in Abu Dhabi who took care of an earlier laboratory-confirmed patient. Of these, two cases, a 28-year-old man and 30-year-old woman, did not develop symptoms of illness. The other two cases, both women of 30 and 40 years old, had mild upper respiratory symptoms and are in stable condition.

Globally, from September 2012 to date, WHO has been informed of a total of 88 laboratory-confirmed cases of infection with MERS-CoV, including 45 deaths.

Based on the current situation and available information, WHO encourages all Member States to continue their surveillance for severe acute respiratory infections (SARI) and to carefully review any unusual patterns.

Health care providers are advised to maintain vigilance. Recent travelers returning from the Middle East who develop SARI should be tested for MERS-CoV as advised in the current surveillance recommendations.

Specimens from patients’ lower respiratory tracts should be obtained for diagnosis where possible. Clinicians are reminded that MERS-CoV infection should be considered even with atypical signs and symptoms, such as diarrhea, in patients who are immunocompromised.

Health care facilities are reminded of the importance of systematic implementation of infection prevention and control (IPC). Health care facilities that provide care for patients suspected or confirmed with MERS-CoV infection should take appropriate measures to decrease the risk of transmission of the virus to other patients, health care workers and visitors.

All Member States are reminded to promptly assess and notify WHO of any new case of infection with MERS-CoV, along with information about potential exposures that may have resulted in infection and a description of the clinical course. Investigation into the source of exposure should promptly be initiated to identify the mode of exposure, so that further transmission of the virus can be prevented.

WHO does not advise special screening at points of entry with regard to this event nor does it currently recommend the application of any travel or trade restrictions.

WHO has convened an Emergency Committee under the International Health Regulations (IHR) to advise the Director-General on the status of the current situation. The Emergency Committee, which comprises international experts from all WHO Regions, unanimously advised that, with the information now available, and using a risk-assessment approach, the conditions for a Public Health Emergency of International Concern (PHEIC) have not at present been met.

A novel clade 2c betacoronavirus, termed Middle East respiratory syndrome (MERS)–CoV, was recently identified as the causative agent of a severe respiratory disease that is mainly affecting humans on the Arabian Peninsula (1). Extending on previous work (2), we described European Pipistrellus bat–derived CoVs that are closely related to MERS-CoV (3). We now report the identification of a South Africa bat derived CoV that has an even closer phylogenetic relationship with MERS-CoV.

1 AUGUST 2013 - WHO has been informed of an additional three laboratory-confirmed cases of Middle East respiratory syndrome coronavirus (MERS-CoV) infection in Saudi Arabia.

The first patient is a 67-year-old woman from Riyadh with underlying medical conditions. She became ill on 25 July 2013. She has no known exposure to animals or to a case confirmed with MERS-CoV infection. She is currently hospitalized.

The other two patients are health care workers, both women, from Assir and Riyadh regions . Both of them have mild symptoms and were exposed to patients who were laboratory-confirmed cases.

Artikel 3
Artikel 2, tweede lid, van de Regeling publieke gezondheid komt te luiden:2.Voor de meldingsplicht van de arts op grond van artikel 22, eerste lid, van de wet, geldt voor het Middle East respiratory syndrome coronavirus (MERS-CoV) het volgende: de meldings-plicht wordt beperkt tot de vaststelling van een infectie bij een persoon, die op grond van de ernst van dit ziektebeeld is opgenomen in een ziekenhuis, door de behandelend arts van het ziekenhuis.

Artikel 4
Deze regeling treedt in werking met ingang van de dag na de datum van uitgifte van de Staatscourant waarin zij wordt geplaatst.

Artikel 5
Deze regeling wordt aangehaald als: Regeling MERS-CoV.

The Supreme Council of Health (SCH) has reported a new Middle East Respiratory Syndrome corona virus case.

The patient is a 29-year-old Qatari citizen who has a exposure to a confirmed case and suffers from asthma and several risk factors.

SCH said in a press release that the patient was checked up and diagnosed with the disease in the National Influenza Laboratory, adding that samples were also sent to reference labs abroad to confirm the diagnosis.

The statement added that the patient, who is in a critical condition, is receiving treatment in the intensive care unit.

28 August 2013 - WHO has been informed of an additional eight laboratory-confirmed cases of Middle East respiratory syndrome coronavirus (MERS-CoV) infection in Saudi Arabia.

The cases include two men with underlying medical conditions from Riyadh who died, three women and one man, also from Riyadh, and two men from Asir region. Additionally, a previously laboratory-confirmed case from the United Arab Emirates has died.

The two cases who died were 54 and 51 years-old from Riyadh, who were suffering from pre-existing chronic illnesses. The other patients include:

    a 50-year-old woman with an underlying medical condition, from Riyadh, who became ill on 1 August 2013 and is currently hospitalized, now off mechanical ventilation;
    a 59-year-old woman with underlying medical conditions, from Riyadh, who became ill on 23 July 2013 and is currently hospitalized in intensive care unit and is in critical condition;
    a 50-year-old woman with underlying medical conditions, from Riyadh, who is currently hospitalized in intensive care unit;
    a 70-year-old man with underlying medical conditions, from Riyadh, who is currently hospitalized in intensive care unit;
    a 31-year-old man with underlying medical conditions, from Asir, who is currently hospitalized in intensive care unit; and
    a 55-year-old man from Asir who is a contact of a confirmed case, and is asymptomatic.

Globally, from September 2012 to date, WHO has been informed of a total of 102 laboratory-confirmed cases of infection with MERS-CoV, including 49 deaths.

4 September 2013 – WHO has been informed of an additional two laboratory-confirmed cases of infection with Middle East respiratory syndrome coronavirus (MERS-CoV).

The first case was notified by the Ministry of Health in Tunisia. The patient was a 66-year-old man who became ill on 1 May 2013 and died on 10 May 2013. The patient was earlier announced as a probable case while his daughter and son were laboratory-confirmed with MERS-CoV (DON published on 22 May 2013).  Laboratory-confirmation on the case was recently conducted by the US Centers for Disease Control and Prevention.

The second case was notified by the Ministry of Health in Qatar.  The patient was a 56-year-old woman with underlying medical conditions who became ill on 18 August 2013 and died on 31 August 2013.  Laboratory-confirmation was recently conducted by Public Health England, UK.

Globally, from September 2012 to date, WHO has been informed of a total of 110 laboratory-confirmed cases of infection with MERS-CoV, including 52 deaths.

7 SEPTEMBER 2013 - WHO has been informed of four (4) additional laboratory-confirmed cases of Middle East respiratory syndrome coronavirus (MERS-CoV) infection in Saudi Arabia.

The first case is a 41 year-old female healthcare worker from Riyadh with no known underlying medical conditions and who became ill on 15 August, 2013. Her condition deteriorated and she passed away at the end of August. No known exposure to animals, or to a confirmed MERS-CoV case, has been identified and investigations into the source of infection are on-going.

The second case is a 30 year-old Saudi male healthcare worker from Riyadh, working in the same hospital as the above confirmed case. He developed severe pneumonia on 1 September, 2013, and is currently in critical condition.

The third case is a 79 year-old woman from Hafar al-Batin province who developed a respiratory illness on 21 August, 2013. She is a contact of a confirmed MERS-CoV case in a family cluster. Her condition deteriorated and she passed away on 2 September, 2013.

The fourth case is a 47 year-old Saudi man from Hafar al-Batin province with a chronic heart condition and who became ill on 23 August, 2013. He is a contact of a confirmed MERS-CoV case in a family cluster. He is currently in critical condition.

All Member States are reminded to promptly assess and notify WHO of any new case of infection with MERS-CoV, along with information about potential exposures that may have resulted in infection and a description of the clinical course. Investigation into the source of exposure should promptly be initiated to identify the mode of exposure, so that further transmission of the virus can be prevented.

WHO does not advise special screening at points of entry with regard to this event nor does it currently recommend the application of any travel or trade restrictions.

A separate infection called Mers-coronavirus, which is centred on Saudi Arabia, has infected 114 people and killed 54 deaths.
Researchers at the University of Madrid have created a mutated version of the virus, which could be the first steps towards creating a vaccine.
Their study in the journal, mBio, shows the modified virus can infect cells but struggles to spread round the body.

19 September 2013 - WHO has been informed of an additional 18 new laboratory-confirmed cases including three deaths with Middle East respiratory syndrome coronavirus (MERS-CoV) infection in Saudi Arabia.

The patients are reported from Hafar Al-Batin, Medina and Riyadh. Their ages ranging from three to 75 years old. These cases were announced by the Ministry of Health in Saudi Arabia on 1, 5, 8, 10 and 11 September 2013.

Additionally, in Qatar, a previously laboratory-confirmed patient with MERS-CoV died on 6 September 2013.

The third meeting of the Emergency Committee convened by the Director-General under the International Health Regulations (2005) [IHR (2005)] was held by teleconference on Wednesday, 25 September 2013, from 12:00 to 14:30 Geneva time (CET).

During the informational session, Kingdom of Saudi Arabia and Qatar presented on recent developments in their countries. The WHO Secretariat provided an update on epidemiological developments, Hajj and Umrah and recent WHO activities related to MERS-CoV. The Committee reviewed and deliberated on the information provided.

The Committee concluded that it saw no reason to change its advice to the Director-General. Based on the current information, and using a risk-assessment approach, it was the unanimous decision of the Committee that the conditions for a Public Health Emergency of International Concern (PHEIC) have not at present been met.

While not considering the events to constitute a PHEIC, Members of the Committee reiterated their prior advice for consideration by WHO and Member States and emphasized the importance of:

- strengthening surveillance, especially in countries with pilgrims participating in Umrah and the Hajj;
- continuing to increase awareness and effective risk communication concerning MERS-CoV, including with pilgrims;
- supporting countries that are particularly vulnerable, especially in Sub-Saharan Africa taking into account the regional challenges;
- increasing relevant diagnostic testing capacities;
- continuing with investigative work, including identifying the source of the virus and relevant exposures through case control studies and other research; and
  timely sharing of information in accordance with the International Health Regulations (2005) and ongoing active coordination with WHO.

The WHO Secretariat will continue to provide regular updates to the Members, and currently anticipates reconvening the Committee in late November 2013. Any serious new developments may require re-convening the Committee before then. Based on these views and the currently available information, the Director-General accepted the Committee’s assessment and thanked its Members for their advice.

The patient is a 61-year-old man with underlying medical conditions who was admitted to a hospital on 11 October 2013. He is currently hospitalized and is in a stable condition. The patient was tested positive for MERS-CoV infection in Qatar and was confirmed by the reference laboratory of Public Health England yesterday.

Preliminary investigations revealed that the patient had not travelled outside Qatar in the two weeks prior to becoming ill. The patient owns a farm and has had significant contact with the animals, including camels, sheep and hens. Some of the animals in his farm have been tested and were negative for MERS-CoV. Further investigations into the case and the animals in the farm are ongoing.

Globally, from September 2012 to date, WHO has been informed of a total of 139 laboratory-confirmed cases of infection with MERS-CoV, including 60 deaths.

Supreme Council of Health Qatar reports new Corona virus case
DOHA, Oct 17 -  Qatar confirmed a new Corona virus case. A 61-year-old male Qatari national has been diagnosed in the National Influenza Centre at Hamad Medical Corporation with Middle East Respiratory Syndrome (MERS-CoV).
The patient is in stable condition in the intensive care unit of the Hamad General Hospital.
The SCH’s statement (in Arabic) also says the patient, who suffers with chronic illnesses, didn’t travel for two weeks prior to the infection. He had no known exposure to animals or a confirmed MERS-CoV case.
The SCH noted that all family members of the patient were examined and preliminary results were negative.
With this new case, it brings the global total of MERS-CoV cases in Qatar to 6, with 2 cases ending in death.

The Supreme Council of Health and the Ministry of Environment in collaboration with the National Institute of Public Health and Environment (RIVM) of the Ministry of Health and the Erasmus Medical College in the Netherlands announced confirmation of the first case of Middle East Respiratory Syndrome Coronavirus (MERS-CoV) in 3 camels in a herd in Qatar in a barn, which is linked to two confirmed human cases who have since then recovered.

For transparency purposes, we can confirm that the 3 camels were investigated among a herd of 14 camels, and the samples were collected as part of the epidemiological investigation in coordination between the Public Health Department and the Department of Animal Resources. It is to be noted that none of the 14 camels showed any sign of disease when the samples were collected. As a precautionary measure, the 14 camels were put in quarantine since the initial sampling and after 40 days as of now, none have shown any symptom or sign of the disease.

For information, the presence of the MERS-CoV is newly recognized among animals, and currently there is neither clear scientific case definition nor enough information as to the role animals may play in transmitting and spreading the diseases.

All contacts of the two recovered MERS-CoV cases, including relatives, friends and workers in the same barn have been screened with negative results. The two Departments are following up with the reference laboratory and Erasmus Medical College to test additional samples from other animal species and from the environment of the barn. The joint team of the Supreme Council of Health and the Department of Animal Resources is continuously monitoring the development of this disease and taking all necessary measures to prevent the spread of the disease.

This discovery came as a result of the collaborative efforts between the two ministries, and the RIVM laboratory and Erasmus Medical College in the Netherlands, together with the World Health Organization (WHO). Currently the two Departments are conducting a national survey to investigate the presence of virus in animals, humans and the environment, and the potential modes of transmission and exposure to the virus among humans who are in close contacts with animals. Until more information is available, it is recommended, that as a precautionary measure, any animals that have been in close contact with newly detected human MERS-CoV cases are separated for investigation of the presence of infection with the virus.

It is also recommended that people with underlying health conditions, such as heart disease, diabetes, kidney disease, respiratory disease, the immunosuppressed, and the elderly, avoid any close animal contacts when visiting farms and markets, and to practice good hygiene, such as washing hands

A 32-year-old woman in Abu Dhabi infected with the virus has died; her husband and son are also diagnosed with MERS.
A 32-year-old Jordanian woman in Abu Dhabi diagnosed with the Middle East respiratory syndrome coronavirus (MERS-CoV) has died, Health Authority Abu Dhabi (HAAD) confirmed.

A joint Qatari and Dutch research team has succeeded in culturing the Middle East Respiratory Syndrome Coronavirus (Mers-CoV) in the laboratory, the Supreme Council of Health (SCH) announced yesterday.
The breakthrough will increase the knowledge for prevention and control of Mers-CoV in addition to enhancing the development of diagnostic tools and production of vaccines and treatment for the virus.
The achievement is the result of combined efforts from the SCH and the Ministry of Environment from Qatar as well as Erasmus Medical Centre in the Netherlands with support of the World Health Organisation (WHO).

The SCH has pointed out that in Qatar the efforts continue to determine the source and root of the virus transmission, resulting in a series of studies, one of which yielded the virus.

Based on WHO’s recommendations, a national sero-survey will be conducted by the end of this month, to determine risk factors for MERS-CoV as well as to identify the source of the virus. The sero-survey will include screening of animals and workers.

The collaboration between Qatar, Erasmus Medical Centre and the National Institute for Public Health and the Environment of the ministry of health in the Netherlands focuses on research co-operation and exchange of experiences and expertise as well as training of laboratory technicians for the diagnosis of the virus.

This resulted in matching laboratory results between veterinary biotechnology laboratories under Qatar’s Ministry of Environment with the Erasmus Medical Centre in the Netherlands in virus detection by using the polymerase chain reaction (PCR) techniques, making the labs in Qatar the first in the Middle East for the diagnosis of Mers-CoV in animals.

The SCH also announced that there had been no new human cases of the Mers-CoV in Qatar since October 2013, although intensive  surveillance of the disease is undergoing in collaboration between healthcare providers in hospitals and Department of Animal Health resources.

The SCH has urged all citizens and residents to apply measures for the prevention of the disease and to communicate with healthcare providers if they develop acute respiratory symptoms, especially if accompanied by fever and there was a close contact with animals.

Machine translations of a statement from the Saudi Ministry of Health (MOH) cited 11 MERS cases in Jeddah, including 6 at King Fahd Hospital. It appears that at least some of these were announced previously.

The King Fahd emergency department was closed for 24 hours for disinfection, according to a machine translation posted today on Avian Flu Diary, an infectious disease news blog. It said three contacts of MERS patients at the hospital have tested positive for the virus and are sick, while others have tested positive but have no symptoms.

"This brings the number of confirmed cases and [those] previously announced in the governorate of Jeddah to 11, of whom 2 have died, 6 have recovered, and 3 are still receiving medical care," said an edited translation of the MOH statement provided by ProMED-mail, the disease reporting service of the International Society for Infectious Diseases.

A King Fahd Hospital official said six of the patients were at King Fahd, two were at King Abdul Aziz Hospital, and three others, all doctors, were at three other hospitals, according to a Gulf News report today. The other hospitals are King Faisal, University, and National Guard. The report was based on a story in the newspaper Al Madinah.

UAE-based paramedic dies after contracting MERS virus
A Filipino ambulance paramedic who works for the UAE Interior Ministry has died after contracting the MERS coronavirus.
The ministry also announced that five others, also Filipinos, have been infected with the virus, news agency WAM reported.
Officials said in a statement that the five paramedics were found to be infected with the virus after a routine check-up.
They have been placed in quarantine and the ministry has contacted the people who were treated by the paramedics so they can undergo a precautionary check.

14 APRIL 2014 - WHO has been informed of an additional 16 laboratory-confirmed cases of infection with Middle East respiratory syndrome coronavirus (MERS-CoV) from Saudi Arabia and the United Arab Emirates (UAE).

The 15 additional laboratory-confirmed cases, including two deaths announced on the Ministry of Health of Saudi Arabia website and provided to WHO between 6 and 11 April include:
A 70 year-old man from Jeddah with underlying medical conditions. He became ill on 24 March, was hospitalized on 29 March and died on 5 April.
A 28 year-old man from Jeddah with no reported symptom of illness. The man is a household contact of the above mentioned laboratory-confirmed case.
Three health-care workers – a 26 year-old woman, a 26 year-old man and a 33 year-old man with no symptoms of illness.
A 28 year-old man who is a health-care worker in Jeddah. He became ill on 28 March, was admitted to a hospital on 3 April and is currently receiving treatment in an intensive care unit.
A 35 year-old man from Jeddah with no reported symptom of illness.
A 32 year-old woman from Jeddah who is a health-care worker with no reported symptom of illness.
A 45 year-old man from Riyadh. He became ill on 30 March, was hospitalized on 5 April and is currently receiving treatment in an intensive care unit. He had no history of exposure to animals nor contact with a laboratory-confirmed case.
A 90 year-old man from Riyadh. He became ill on 30 March, was hospitalized on 1 April, and is currently receiving treatment in an intensive care unit. He had no history of exposure to animals nor contact with a laboratory-confirmed case.
A 57 year-old man from Riyadh with underlying medical conditions. He became ill on 16 March, was admitted to a hospital on 19 March and died on 30 March.
Four men aged 29, 33, 34 and 70 years old from Jeddah.
Additionally, a previously laboratory-confirmed case has died. The concerned health authorities in Saudi Arabia are currently conducting investigations into the contacts of the cases.

The additional laboratory-confirmed case reported by the Ministry of Health of the UAE on 10 April 2014 includes:
A 45 year-old man from Abu Dhabi who became ill on 6 April, was hospitalized on 7 April and died on 10 April. The patient was not known to have any chronic disease. He did not have a recent history of travel or contact with animals or with a previously laboratory-confirmed case.
The concerned health authorities in the UAE are conducting investigations into the contacts of the case.

Globally, from September 2012 to date, WHO has been informed of a total of 228 laboratory-confirmed cases of infection with MERS-CoV, including 92 deaths.

WHO advice

Based on the current situation and available information, WHO encourages all Member States to continue their surveillance for severe acute respiratory infections (SARI) and to carefully review any unusual patterns.

Infection prevention and control measures are critical to prevent the possible spread of MERS-CoV in health care facilities. Health-care facilities that provide for patients suspected or confirmed to be infected with MERS-CoV infection should take appropriate measures to decrease the risk of transmission of the virus from an infected patient to other patients, health-care workers and visitors. Health care workers should be educated, trained and refreshed with skills on infection prevention and control.

It is not always possible to identify patients with MERS-CoV early because some have mild or unusual symptoms. For this reason, it is important that health-care workers apply standard precautions consistently with all patients – regardless of their diagnosis – in all work practices all the time.

Droplet precautions should be added to the standard precautions when providing care to all patients with symptoms of acute respiratory infection. Contact precautions and eye protection should be added when caring for probable or confirmed cases of MERS-CoV infection. Airborne precautions should be applied when performing aerosol generating procedures.

Patients should be managed as potentially infected when the clinical and epidemiological clues strongly suggest MERS-CoV, even if an initial test on a nasopharyngeal swab is negative. Repeat testing should be done when the initial testing is negative, preferably on specimens from the lower respiratory tract.

Health-care providers are advised to maintain vigilance. Recent travellers returning from the Middle East who develop SARI should be tested for MERS-CoV as advised in the current surveillance recommendations. All Member States are reminded to promptly assess and notify WHO of any new case of infection with MERS-CoV, along with information about potential exposures that may have resulted in infection and a description of the clinical course. Investigation into the source of exposure should promptly be initiated to identify the mode of exposure, so that further transmission of the virus can be prevented.

People at high risk of severe disease due to MERS-CoV should avoid close contact with animals when visiting farms or barn areas where the virus is known to be potentially circulating. For the general public, when visiting a farm or a barn, general hygiene measures, such as regular hand washing before and after touching animals, avoiding contact with sick animals, and following food hygiene practices, should be adhered to.

WHO does not advise special screening at points of entry with regard to this event nor does it currently recommend the application of any travel or trade restrictions.

Saudi Arabia fires health chief amid deadly virus surge

Saudi Arabia has replaced its health minister as the kingdom grapples with a surge of new cases of the incurable SARS-like Middle East Respiratory Syndrome (MERS), a viral disease that kills 30 to 50 percent of the people it infects.

The Saudi government has reported 244 MERS cases – 79 of which have been fatal – since the disease was identified in 2012. Authorities announced 49 confirmed new infections over the past week, mostly in the port city of Jeddah, a 25 percent increase in the total number of confirmed cases since the outbreak began. Seven people in Jeddah have already died.

CDC: First US case of deadly virus MERS reported in Indiana
MUNSTER, Ind. (May 2, 2014) — The first U.S. case of MERS-CoV has been reported in Indiana, the Centers for Disease Control and Prevention said Friday. The patient is currently being treated at Community Hospital in Munster.
MERS-CoV, short for Middle East Respiratory Syndrome, is a type of coronavirus. The incubation period for MERS is 5 days.
CDC  and the Indiana State Department of Heath are conducting a joint investigation of the first case of MERS-CoV in the United States. MERS-CoV, a virus new to humans, was first reported in the Arabian Peninsula in 2012.

A team of experts from WHO started a two-day mission yesterday in Jeddah to assist national health authorities to investigate the recent increase in number of people infected by MERS-CoV.

From mid-March 2014, 111 people have tested positive in the Jeddah area; the biggest single surge in the MERS-CoV outbreak since the new virus was detected in April 2012. Thirty-one persons have died.

As a large proportion of infections in Jeddah occurred in health-care facilities, the WHO team began with analyzing transmission patterns in the city’s main hospitals.

“We need to understand how people got infected in health-care settings, and in the community; we are looking into possible infection routes and whether the virus has changed its ability to more easily infect people,” says Dr Jaouad Mahjour, WHO Team Leader, “but we know that the systematic application of basic infection prevention and control measures in health facilities is key to limiting transmission and protecting health-care workers and other patients.”

Following the confirmation of the first cases, health facilities increased laboratory testing of patients, close contacts and health-care workers and strengthened protective measures. One third of the people tested positive in the recent spate of cases are health-care workers with mild or no symptoms.

“Our priority is to stop the transmission inside the hospital by strengthening infection prevention and control activities,” says Dr Mohammed Al Ghamdi, an Infectious Disease Consultant at the King Fahd Hospital, the city’s main general hospital - where 78 people have tested positive so far. “WHO is helping us in getting answers on transmission routes not only in health facilities, but also in the community.”
Primary source of infection

Camels are increasingly being seen as the primary source of the MERS-CoV infecting humans, as recent studies showed that camels can harbour live virus. However, it is still unclear how exactly the virus is being transmitted to humans. Once infected, the person can pass the virus to other people, but no sustained chains of infection have been observed.

“Until we better understand how the virus transmits from camels or the environment to a human, we are likely to see more cases, and with travel, the virus is exportable,” said Dr. Mahjour. “Therefore, understanding this link is key to limiting the outbreak. There is an urgent need to conduct an in-depth epidemiological study of known cases to get this knowledge.”

Based on the current situation and available information, WHO encourages all Member States to continue their surveillance for severe acute respiratory infections (SARI) and to carefully review any unusual patterns.

Until further information is gathered, it is prudent for persons at high risk of severe disease due to MERS-CoV (including those with diabetes, chronic lung disease, pre-existing renal failure, or those who are immunocompromised) to take appropriate precautions when visiting farm and market environments where camels are present. These measures might include avoiding contact with camels, good hand hygiene, and avoiding drinking raw milk or eating food that may be contaminated with animal secretions or products unless they are properly washed, peeled, or cooked.

For the general public, when visiting a farm, live animal markets or a barn, general hygiene measures, such as regular hand washing before and after touching animals, avoiding contact with sick animals, and following food hygiene practices, should be adhered to.

WHO does not advise special screening at points of entry with regard to this event nor does it currently recommend the application of any travel or trade restrictions.

First US MERS patient worked in Saudi Arabia healthcare
A person who recently worked in healthcare in Saudi Arabia and fell ill after flying to the United States on Apr 24 has the first confirmed US case of MERS-CoV (Middle East respiratory syndrome coronavirus), federal health officials announced today while asserting that there is very little risk to the public.

The patient, who was not identified, was admitted to Community Hospital in Munster, Ind., on Apr 28, tested positive for MERS this afternoon, and is in stable condition, the Centers for Disease Control and Prevention (CDC) announced. Alert over spike in Middle East flu virus
DUBAI, 5 May 2014 (IRIN) - Health authorities around the world are on high alert after a sudden spike in the incidence of a deadly Middle Eastern flu bug that began in Saudi Arabia but has now spread to Asia and the USA.

As of 4 May, the Kingdom had recorded 411 cases of Middle East Respiratory Syndrome Coronavirus (MERS-CoV) and 112 deaths in two years.

Almost a third of these cases have emerged in the past two weeks, the biggest single surge of MERS-CoV since it was discovered in April 2012, prompting questions about whether the virus has the potential to escalate into a pandemic similar to Severe Acute Respiratory Syndrome (SARS), which swept through Asia in 2002-3, killing over 700 people.

Beyond Saudi Arabia the virus has a significant caseload in the United Arab Emirates (UAE) and in the past week other cases have been discovered in the US, Greece and Egypt for the first time, with 12 countries in total now affected.

WHO concludes MERS-CoV mission in Saudi Arabia
Cairo, Egypt 7 May, 2014 – A team of experts from the World Health Organization (WHO) completed a 5-day mission to Saudi Arabia to assist the national health authorities to assess the recent increase in the number of people infected by the Middle East respiratory syndrome coronavirus (MERS-CoV) in Jeddah.
As of 3 May, 489 cases, including 126 deaths, were reported to WHO globally and 406 cases, including 101 deaths, from Saudi Arabia. These numbers can change from one day to the next according to when Member States inform WHO.
The team looked into the epidemiological, disease prevention, organizational and communication aspects of this recent outbreak to understand the public health risk and transmission chain and to propose next steps and actions.
After meeting health officials in the capital, WHO experts visited two main hospitals in Jeddah to analyse transmission patterns and review infection control measures.
Key findings of the Jeddah outbreak include the following.
Current evidence does not suggest that a recent increase in numbers reflects a significant change in the transmissibility of the virus. The upsurge in cases can be explained by an increase, possibly seasonal, in the number of primary cases amplified by several outbreaks in hospitals due to breaches in WHO’s recommended infection prevention and control measures. There is no evidence of sustained human-to-human transmission in the community and the transmission pattern overall remained unchanged.
The majority of human-to-human infections occurred in health care facilities. One quarter of all cases have been health care workers.  There is a clear need to improve health care workers’ knowledge and attitudes about the disease and systematically apply WHO’s recommended infection prevention and control measures in health care facilities.
 The reasons for the increase in the number of primary community cases, as well as the infection route, remain unknown. Three quarters of all primary community cases have been male, the majority of whom have been over 50 years old. Secondary transmission in the community and households is much lower than in health care settings.
Some confirmed cases presented with mild or no symptoms.
Based on the current situation and available information, WHO encourages all Member States to continue their surveillance for severe acute respiratory infections and to carefully review unusual patterns.
WHO does not advise special screening at points of entry with regard to this event nor does it currently recommend the application of any travel or trade restrictions, including for upcoming pilgrimage travel to Saudi Arabia.

Second U.S. case of MERS virus found in Florida, CDC says
A second case of the deadly MERS virus has been confirmed in the United States, the Centers for Disease Control and Prevention announced today.
The CDC said the disease was diagnosed Sunday in a patient at an Orlando, Florida, hospital. They said the man, from Saudi Arabia, was in the area visiting relatives when he fell ill last week. They noted that he had not visited at any Orlando-area theme parks.
This man -- like the first U.S. patient in Indiana earlier this month -- was a health care worker in a hospital in Saudi Arabia where MERS cases have been treated. Officials said the two U.S. cases are not related.

MERS-CoV -- which stands for Middle East respiratory syndrome coronavirus -- was first identified in Saudi Arabia in 2012. Since then at least 480 cases have been confirmed in Saudi Arabia and 139 people there have died. Cases have also been reported in several nearby countries including the United Arab Emirates, Egypt, Jordan and Lebanon.
The only cases identified so far in the U.S. or Europe have been in people who had recently traveled to the region.
Symptoms of MERS can include fever, breathing problems, pneumonia and kidney failure.

Eerste MERS-patiënt in Nederland
Op 13 mei hebben het RIVM en het Erasmus MC voor het eerst een infectie met het MERS-coronavirus vastgesteld bij een Nederlandse patiënt. Hij raakte besmet tijdens een bezoek aan Saoedi-Arabië en is op dit moment opgenomen in het Medisch Centrum Haaglanden, locatie MCH Westeinde. Hier wordt de patiënt in strikte isolatie verpleegd. Zijn gezondheidstoestand is stabiel. Alle mensen die contact hebben gehad met de patiënt worden in kaart gebracht. Van deze personen wordt de gezondheidstoestand in de gaten gehouden.

MERS-infecties kunnen ernstige luchtwegklachten veroorzaken, vooral bij mensen met andere gezondheidsproblemen. Ook kunnen diarreeklachten voorkomen. Mens-op-mensoverdracht van het MERS-coronavirus komt weinig voor, behalve bij nauw contact met patiënten en bij onbeschermde verpleging van ernstig zieke personen.

Sinds 2012 is er een uitbraak van het nieuwe MERS-coronavirus (Middle East Respiratory Syndrome) in het Midden-Oosten. Inmiddels zijn ruim 500 patiënten in het Midden-Oosten gemeld bij de Wereldgezondheidsorganisatie. In april en mei 2014 is er een forse toename in het aantal meldingen in het Midden-Oosten. Verreweg de meeste meldingen komen uit deze regio (vooral Saoedi-Arabië, maar ook Qatar, Jordanië, Oman, Jemen en Verenigde Arabische Emiraten). Sinds vorig jaar zijn er ook enkele patiënten vastgesteld in het Verenigd Koninkrijk, Frankrijk, Italië, Griekenland, Filippijnen, Maleisië, Koeweit, Tunesië, Duitsland, Egypte en de Verenigde Staten. Dit waren reizigers die besmet zijn geraakt in het Midden-Oosten. Het is niet onverwachts dat er nu ook een patiënt in Nederland is. MERS-coronavirus was daarom al een meldingsplichtige infectieziekte.

Het Medisch Centrum Haaglanden is voor opnames en afspraken geopend voor patiënten en bezoekers.

The fifth meeting of the Emergency Committee convened by the Director-General under the International Health Regulations (2005) concerning Middle East respiratory syndrome coronavirus (MERS-CoV) was held by teleconference on Tuesday, 13 May 2014, from 12:07 to 17:12 Geneva time (CEST).

In addition to Members of the Emergency Committee, three expert advisors participated in the informational session only1. These advisors did not participate in the formulation of advice to the Director-General.

Thirteen affected States Parties reporting cases of MERS-CoV or evidence of infection since December 2013, were also on the first part of the teleconference: Egypt, Greece, Jordan, Kuwait, Lebanon, Malaysia, Oman, Philippines, Qatar, Saudi Arabia, United Arab Emirates, United States of America, and Yemen.

The WHO Secretariat provided an update on and assessment of epidemiological and scientific developments, including a description of the recent increase in cases in communities and in hospitals, transmission patterns, and the main observations of a WHO mission to Saudi Arabia, conducted 28 April – 5 May 2014.

Affected countries gave information about recent events in their countries, including measures taken and their concerns about the current situation.

The Members of the Committee discussed the information provided. Based on current information, the Committee indicated that the seriousness of the situation had increased in terms of public health impact, but that there is no evidence of sustained human-to-human transmission. As a result of their deliberations, the Committee concluded that the conditions for a Public Health Emergency of International Concern (PHEIC) have not yet been met.

However, the Committee emphasized that its concern about the situation had significantly increased. Their concerns centred on the recent sharp rise in cases; systemic weaknesses in infection prevention and control, as well as gaps in critical information; and possible exportation of cases to especially vulnerable countries. The Committee strongly urged WHO and Member States to take immediate steps to:

    improve national policies for infection prevention and control, and implement them in health-care facilities in all countries; this is most urgent for affected countries;
    initiate and accelerate critical investigations, including case-control, serological, environmental, and animal studies, to better understand the epidemiology, especially risk factors and assess the effectiveness of control measures;
    support countries that are particularly vulnerable, especially in sub-Saharan Africa, taking into account the regional challenges;
    strengthen case and contact identification and management;
    greatly enhance awareness and effective risk communication concerning MERS-CoV to the general public, health professionals, at-risk groups, and policy makers;
    strengthen intersectoral collaboration and information sharing across ministries and with relevant international organizations, especially with the World Organization for Animal Health (OIE) and the Food and Agriculture Organization of the United Nations (FAO);
    develop and disseminate advice regarding mass gatherings to prevent further spread of MERS-CoV;
    share information in a timely manner with WHO, in accordance with the International Health Regulations (2005).

Based on the Committee’s advice, and information currently available, the Director-General accepted the Committee’s assessment. She thanked the Committee for its work.

The WHO Secretariat will continue to provide regular updates to the Committee Members and Advisors. In view of the Committee’s concerns, the Emergency Committee will be reconvened in June 2014 or earlier if circumstances require.

Sinds vorig jaar zijn er ook enkele patiënten vastgesteld in het Verenigd Koninkrijk, Frankrijk, Italië, Griekenland, Filippijnen, Maleisië, Koeweit, Tunesië, Duitsland, Egypte en de Verenigde Staten. Dit waren reizigers die besmet zijn geraakt in het Midden-Oosten. Het is niet onverwachts dat er nu ook een patiënt in Nederland is. MERS-coronavirus was daarom al een meldingsplichtige infectieziekte.

DEN HAAG - De opname van een patiënt met het gevaarlijke MERS-virus in het MCH Westeinde in Den Haag heeft nauwelijk tot onrust geleid. Volgens GGD Haaglanden is er welgeteld één telefoontje binnengekomen over het virus.

Zorgen over MERS nemen toe
De zorgen over het MERS-coronavirus nemen toe, maar de WHO wil nog niet spreken van een officiële, internationale bedreiging voor de volksgezondheid. Dit was de uitkomst van een bijeenkomst van de Emergency Committee van de WHO, die deze week plaatsvond.

Het aantal MERS-CoV-gevallen is recentelijk fors gestegen: van de ongeveer 500 gevallen die wereldwijd gemeld zijn, is bijna de helft in de afgelopen maand vastgesteld. In Nederland is de eerste patiënt met de ziekte geconstateerd.

Het comité besloot nog geen ‘public health emergency of international concern’ (PHEIC) uit te roepen. Als dat zou gebeuren, kan de WHO maatregelen nemen, zoals het geven van (negatieve) reisadviezen. Op dit moment geeft de WHO alleen adviezen aan de betrokken landen, waar sprake is van onvoldoende infectiepreventie- en controle. Speciale aandacht is nodig voor landen waar de gezondheidszorg ‘kwetsbaar’ is, zoals het mooi heet. Daarbij wordt gedacht aan bijvoorbeeld sub-Sahara-landen. Als MERS naar die landen wordt geëxporteerd, is het zeer onwaarschijnlijk dat de patiënt en mogelijke contacten strikt kunnen worden geďsoleerd, waardoor de ziekte zich verder kan verspreiden.

Vandaag, 15 mei 2014, is bij een tweede Nederlandse patiënt een infectie met het MERS-coronavirus vastgesteld. De vrouw is een familielid van de man waarbij gisteren die diagnose is gesteld. Ook zij is besmet geraakt tijdens dezelfde reis naar Saoedi-Arabië. Zij is opgenomen in Isala te Zwolle en wordt in strikte isolatie verpleegd. Haar gezondheidstoestand is stabiel. Alle mensen die contact hebben gehad met de patiënt worden door de GGD in kaart gebracht. Van deze personen wordt de gezondheidstoestand in de gaten gehouden.


De vrouw heeft in Saoedi-Arabië een bezoek gebracht aan een dromedarissenboerderij. Zij heeft nauw contact met de eerste patiënt gehad, ze deelden gedurende twee weken een hotelkamer. Daarnaast is bekend dat beide patiënten onderliggende aandoeningen hebben die hen waarschijnlijk gevoeliger maken voor een infectie met dit virus.
 De GGD houdt de gezondheidstoestand van alle reisgenoten en van mensen die contact met de patiënten hebben gehad goed in de gaten. Als zij gezondheidsklachten krijgen wordt er onderzoek gedaan. Het is daarom niet uitgesloten dat er in de komende weken bij meer patiënten een infectie met het MERS-coronavirus wordt vastgesteld. Nederlandse ziekenhuizen zijn goed toegerust om deze zorg te bieden.

15 May 2014 - The following cases of laboratory confirmed Middle East respiratory syndrome coronavirus (MERS-CoV) have been reported from Jordan, Lebanon, the Netherlands, the United Arab Emirates, and the United States.

Netherlands

On 14 May 2014, the National IHR Focal Point for the Netherlands notified WHO of the first laboratory confirmed case of MERS-CoV infection in the Netherlands. The patient is a 70 year-old male citizen of the Netherlands, with travel history to the Kingdom of Saudi Arabia between 26 April 2014 and 10 May 2014.

The patient developed first symptoms on 1 May 2014 while in Medina, Saudi Arabia. He was evaluated at an emergency care department in Mecca on 6 May and given antibiotics; he did not have respiratory symptoms while in Saudi Arabia. On return to the Netherlands, on 10 May, his condition deteriorated, including development of respiratory symptoms, and he was hospitalized on the same day. On 13 May, he tested positive for MERS-CoV. Currently, the patient is in the ICU in a stable condition.

The patient reports no contact with animals or consumption of raw animal products. Identification of close contacts, including flight contacts has been initiated.

WHO advice

Based on the current situation and available information, WHO encourages all Member States to continue their surveillance for severe acute respiratory infections (SARI) and to carefully review any unusual patterns.

Infection prevention and control measures are critical to prevent the possible spread of MERS-CoV in health care facilities. Health-care facilities that provide for patients suspected or confirmed to be infected with MERS-CoV infection should take appropriate measures to decrease the risk of transmission of the virus from an infected patient to other patients, health-care workers and visitors. Health care workers should be educated, trained and refreshed with skills on infection prevention and control.

It is not always possible to identify patients with MERS-CoV early because some have mild or unusual symptoms. For this reason, it is important that health-care workers apply standard precautions consistently with all patients – regardless of their diagnosis – in all work practices all the time.

Droplet precautions should be added to the standard precautions when providing care to all patients with symptoms of acute respiratory infection. Contact precautions and eye protection should be added when caring for probable or confirmed cases of MERS-CoV infection. Airborne precautions should be applied when performing aerosol generating procedures.

Patients should be managed as potentially infected when the clinical and epidemiological clues strongly suggest MERS-CoV, even if an initial test on a nasopharyngeal swab is negative. Repeat testing should be done when the initial testing is negative, preferably on specimens from the lower respiratory tract.

Health-care providers are advised to maintain vigilance. Recent travellers returning from the Middle East who develop SARI should be tested for MERS-CoV as advised in the current surveillance recommendations. All Member States are reminded to promptly assess and notify WHO of any new case of infection with MERS-CoV, along with information about potential exposures that may have resulted in infection and a description of the clinical course. Investigation into the source of exposure should promptly be initiated to identify the mode of exposure, so that further transmission of the virus can be prevented.

People at high risk of severe disease due to MERS-CoV should avoid close contact with animals when visiting farms or barn areas where the virus is known to be potentially circulating. For the general public, when visiting a farm or a barn, general hygiene measures, such as regular hand washing before and after touching animals, avoiding contact with sick animals, and following food hygiene practices, should be adhered to.

WHO does not advise special screening at points of entry with regard to this event nor does it currently recommend the application of any travel or trade restrictions

Het CDA in de Tweede Kamer ziet verschil tussen de informatie van het Rijksinstituut voor Volksgezondheid en Milieu (RIVM) en de wereldgezondheidsorganisatie WHO over de uitbraak van de ziekte MERS.

Het Nederlandse RIVM meldt dat het gevaarlijke virus vrijwel alleen van dier op mens kan worden overgedragen, terwijl de VN-organisatie WHO zegt dat het merendeel van de besmettingen van mens op mens zijn gegaan.

Het CDA heeft minister Edith Schippers (Volksgezondheid) zaterdag om opheldering gevraagd. Volgens CDA-Kamerlid Hanke Bruins Slot kan dit verschil van grote betekenis zijn voor de manier waarop omgegaan wordt met besmettingen.

Deze week werd bekend dat de ziekte in Nederland bij twee mensen is vastgesteld. De ziekte dook in 2012 op in Saudi-Arabië. Dromedarissen zijn de bron van het virus.

CDC: Illinois man is 3rd reported case of MERS in nation
A U.S. citizen previously hospitalized in Indiana with Middle East Respiratory Syndrome, or MERS, has passed the potentially fatal virus to an Illinois man, federal health officials said Saturday.

Health officials originally contacted the Illinois resident earlier this month after learning that he had met with the Indiana patient on two occasions prior to the Indiana man’s hospitalization, according to a release from the Centers for Disease Control and Prevention.

An initial test of the Illinois man on May 5 came back negative for an active MERS-CoV infection, officials said. But further testing revealed that the man had in fact been previously infected with the virus.

The Illinois man did not require medical care, officials said, and is reportedly feeling well. Local health officials are continuing to monitor the man's health condition, officials said.

The virus that causes Middle East respiratory syndrome (MERS) has been found in camel milk. Scientists don’t know whether infected milk can sicken people, but experts say the results are reason enough to warn against drinking raw camel milk, a widespread tradition in the Middle East. The National IHR Focal Points of Saudi Arabia, the United Arab Emirates (UAE) and the Islamic Republic of Iran recently reported additional laboratory-confirmed cases of infection with Middle East respiratory syndrome coronavirus (MERS-CoV) to WHO.
Details of the cases reported by Saudi Arabia are as follows:

Between 11 April and 9 June 2014, 515 cases of infection with Middle East respiratory syndrome coronavirus (MERS-CoV) have been reported from Saudi Arabia to WHO. This includes 402 laboratory-confirmed cases reported on various dates, and 113 cases that have been identified through retrospective review of hospital records, and which was reported by Saudi Arabia on 3 June. Further information on these cases will be provided as information becomes available as part of the collaboration between the Saudi authorities and WHO on the MERS-CoV response.

This update covers 402 laboratory-confirmed cases, including 114 deaths.

Thirty-five cases were reported from Madina, 132 from Riyadh, 208 from Mecca Province (including 154 from Jeddah, 39 from Mecca, 8 from Qunfudhah and 7 from Al Taif), 10 from Tabuk, 6 from Al Jawf, 3 from Najran, and 3 from Ash Sharqiyah. The location from where 5 cases were reported was not specified.

The median age of the 402 cases is 46 years old (ranging from 9 months to 94 years old) and 58.3% of those with information on sex (n=388) are men. Almost half (44.5%) of the cases with reported information (n=398) experienced severe disease including 114 cases who died; and 114 cases (28.6%) were reported to be asymptomatic or have mild disease. Underlying medical conditions were only reported for 149 of the 402 cases, of which 140 cases were reported to have at least one underlying medical condition.

More than 25% (109)of the 402 reported cases are health care workers. Among the 109 health care workers, 63 were reported as asymptomatic or developing mild symptoms, 35 were reported with moderate symptoms (requiring hospitalization but not admission to an intensive care unit), 7 were reported as having severe disease and 4 died.
Details of the case reported by the UAE on 4 June 2014 are as follows:

The patient is a 36 year-old butcher residing in Abu Dhabi. He works in a local slaughter house for camels and sheep. He was asymptomatic. His sputum was tested positive for MERS-CoV on 20 May 2014 as part of a general screening in slaughter houses. The patient had no contact with a previously laboratory-confirmed MERS-CoV case. He had no history of travel. The patient is currently isolated and is in a stable condition.

Investigations and follow up of contacts of the patient have been carried out and no other case was detected.
Details of the case reported by the Islamic Republic of Iran on 4 June 2014 are as follows:

The patient is a 35 year-old nurse assistant. She developed a mild illness on 26 May 2014 followed by a productive cough on 28 May 2014. Her throat swab taken on 26 May 2014 was tested positive for MERS-CoV.

She has been advised to stay home and follow infection control precautions. The patient is a close contact to the first laboratory-confirmed MERS-CoV case in the country reported to WHO on 26 May 2014. The patient did not have an underlying medical condition. She had no history of contact with animals and no history of consumption of raw camel products in the 14 days prior to becoming ill. She became asymptomatic on 3 June 2014 and her condition is currently stable.

Investigations into her contacts among health care workers and family members is on-going.

Globally, 697 laboratory-confirmed cases of infection with MERS-CoV including at least 210 related deaths have officially been reported to WHO. This global total includes all the cases in this update; of the abovementioned 402 cases reported by Saudi Arabia, 390 cases have been included in previous Disease Outbreak News (DON) updates published since 14 April 2014.
WHO advice

Based on the current situation and available information, WHO encourages all Member States to continue their surveillance for acute respiratory infections and to carefully review any unusual patterns.

Infection prevention and control measures are critical to prevent the possible spread of MERS-CoV in health care facilities. It is not always possible to identify patients with MERS-CoV early because like other respiratory infections, the early symptoms of MERS-CoV are non-specific. Therefore, health-care workers should always apply standard precautions consistently with all patients, regardless of their diagnosis. Droplet precautions should be added to the standard precautions when providing care to patients with symptoms of acute respiratory infection; contact precautions and eye protection should be added when caring for probable or confirmed cases of MERS-CoV infection; airborne precautions should be applied when performing aerosol generating procedures.

Until more is understood about MERS-CoV, people with diabetes, renal failure, chronic lung disease, and immunocompromised persons are considered to be at high risk of severe disease from MERS‐CoV infection. Therefore, these people should avoid close contact with animals, particularly camels, when visiting farms, markets, or barn areas where the virus is known to be potentially circulating. General hygiene measures such as regular hand washing before and after touching animals and avoiding contact with sick animals, should be adhered to.

Food hygiene practices should be observed. People should avoid drinking raw camel milk or camel urine, or eating meat that has not been properly cooked.

WHO does not advise special screening at points of entry with regard to this event nor does it currently recommend the application of any travel or trade restrictions.

Transmission of MERS-Coronavirus in Household Contacts

Strategies to contain the Middle East respiratory syndrome coronavirus (MERS-CoV) depend on knowledge of the rate of human-to-human transmission, including subclinical infections. A lack of serologic tools has hindered targeted studies of transmission.

Methods
We studied 26 index patients with MERS-CoV infection and their 280 household contacts. The median time from the onset of symptoms in index patients to the latest blood sampling in contact patients was 17.5 days (range, 5 to 216; mean, 34.4). Probable cases of secondary transmission were identified on the basis of reactivity in two reverse-transcriptase–polymerase-chain-reaction (RT-PCR) assays with independent RNA extraction from throat swabs or reactivity on enzyme-linked immunosorbent assay against MERS-CoV S1 antigen, supported by reactivity on recombinant S-protein immunofluorescence and demonstration of neutralization of more than 50% of the infectious virus seed dose on plaque-reduction neutralization testing.

Results
Among the 280 household contacts of the 26 index patients, there were 12 probable cases of secondary transmission (4%; 95% confidence interval, 2 to 7). Of these cases, 7 were identified by means of RT-PCR, all in samples obtained within 14 days after the onset of symptoms in index patients, and 5 were identified by means of serologic analysis, all in samples obtained 13 days or more after symptom onset in index patients. Probable cases of secondary transmission occurred in 6 of 26 clusters (23%). Serologic results in contacts who were sampled 13 days or more after exposure were similar to overall study results for combined RT-PCR and serologic testing.

Conclusions
The rate of secondary transmission among household contacts of patients with MERS-CoV infection has been approximately 5%. Our data provide insight into the rate of subclinical transmission of MERS-CoV in the home.

Statement
1 October 2014

The seventh meeting of the Emergency Committee (EC) convened by the Director-General under the International Health Regulations (IHR 2005) regarding the Middle East respiratory syndrome coronavirus (MERS-CoV) was conducted with members and advisors of the Emergency Committee through electronic correspondence from 26 September 2014 through 30 September 2014.1

The WHO Secretariat provided an update on and assessment of epidemiological and scientific developments, including a description of recently reported cases and transmission patterns. Islamic Republic of Iran and Saudi Arabia provided an update on and assessment of MERS-CoV, including progress towards implementation of the Emergency Committee’s temporary recommendations. 2

The Committee noted that: (i) there have been significant efforts made to strengthen infection prevention and control measures, with an epidemiological situation that has not changed since the 6th meeting of the IHR EC; (ii) the number of cases has fallen since the April upswing, and cases continues to appear sporadically with no evidence of sustained human-to-human transmission in communities; (iii) although transmission in health care settings is still occurring in small clusters, transmission seems generally contained; (iv) activities conducted to reduce the international spread of MERS-CoV seem to be effective; and (v) the current data suggest that MERS-CoV transmission could be seasonal, with an upsurge expected next spring.

The Committee reiterated that its previous advice remains relevant and that significant efforts should be made to:

continue to strengthen infection prevention control (IPC) practices, build capacity of heath-care workers and provide protective equipment in vulnerable countries, especially African countries;
improve awareness about MERS-CoV among pilgrims going for Hajj, and conduct surveillance for MER-CoV among pilgrims during and after Hajj;
harmonise laboratory testing algorithms;
reinforce epidemiological surveillance in camels in the Middle East and in Africa, as well as surveillance in humans and address critical gaps in knowledge of human and animal transmission.
The Committee unanimously concluded that the conditions for a Public Health Emergency of International Concern (PHEIC) have not yet been met.

Based on the Committee’s advice, and information currently available, the Director-General accepted the Committee’s assessment. She thanked the Committee for its work.

The WHO Secretariat will continue to provide regular updates to the Committee Members and Advisors. The Emergency Committee will be reconvened in three months, or earlier if circumstances require.

The Supreme Council of Health (SCH) has announced that a new “Middle East Respiratory Syndrome Corona Virus (MERS-CoV) case has been reported for Qatari patient aged 43 years old. The patient have had fever for few days and was transported by ambulance to the Emergency Department at Hamad General Hospital. Upon assessment and laboratory investigations, the patient was diagnosed with pneumonia and subsequently confirmed as MERS-CoV case. As soon as the Health protection Rapid Response Team got the notification, a series of standard investigation and contact tracing steps were implemented. These include verifying whether contacts have any symptoms and specimen collected. However, all contacts would be monitored for two weeks. The outbreak of Middle East Respiratory Syndrome, or MERS, in the Republic of Korea continues to evolve. WHO is in close contact with the country’s government and Ministry of Health, and is receiving information as soon as facts are confirmed.

The quality of reporting has allowed almost real-time insight into the dynamics of the outbreak. Aggressive contact tracing and testing for infection may help explain the rapid expansion of the outbreak. Human-to-human transmission has been documented. At this stage, WHO has no evidence of sustained transmission in the community.

Korea’s first, or “index”, case was confirmed on 20 May and notified to WHO the same day. The case occurred in a 68-year-old Korean national with a recent history of travel to four countries in the Middle East. The case was asymptomatic during his return flight to Korea on 4 May.

He developed symptoms on 11 May and subsequently sought care at two out-patient clinics and two hospitals, creating multiple opportunities for exposure among health care workers and other patients.

As the index case provided no history of potential exposure to the virus, MERS was not suspected and the patient was not treated in isolation. Efforts are under way to gather more information about exposures during the patient’s travels in the Middle East.

Expansion of the outbreak

The outbreak is the largest reported outside the Kingdom of Saudi Arabia, where the disease first emerged in April 2012 and the vast majority of cases have occurred.

To date, contact tracing has identified a total of 25 laboratory-confirmed cases, including the index case and among health care workers caring for him, patients who were being cared for at the same clinics or hospitals, and family members and visitors.

Some of these additional cases were in the same room as the index case; others were on the same ward. Though data are preliminary, exposure times that led to infection may have been as short as five minutes to a few hours.

To date, two of these cases have been fatal.

Given the number of clinics and hospitals that cared for the index case, further cases can be expected.

On 26 May, one of the confirmed cases with an exposure history in Korea travelled, against medical advice, to Guangdong, China, via Hong Kong. He was symptomatic at the time of travel. On 29 May, China informed WHO that the patient, who was isolated at a Huizhou hospital, tested positive for the MERS coronavirus.

The occurrence of such a large outbreak outside the Middle East is a new development, as is exportation of the disease to a third country. The Ministry of Health has further reported that two recently confirmed cases represent a third generation of transmission – from the index case, to someone exposed to that case, to a third person with no direct exposure to the index case.

Consistent application of adequate measures for infection prevention and control has halted other large clusters of cases associated with health care facilities. WHO recommends that droplet precautions be added to standard measures when providing care to patients with symptoms of acute respiratory infections.

WHO does not advise special screening at points of entry nor does it currently recommend the application of any travel or trade restrictions.

MERS is a newly emerging disease that remains poorly understood. Much about the behaviour of the virus remains shrouded in scientific uncertainty, though evidence is mounting that dromedary camels can transmit the virus to humans through close contact.

WHO is confident that investigations under way in Korea, including the sequencing of viruses and sharing of the findings, will contribute to further scientific understanding of this disease.

Republic of Korea reports the largest MERS-CoV outbreak outside the Arabian Peninsula

The outbreak of Middle East respiratory syndrome coronavirus (MERS-CoV) in the Republic of Korea continues to unfold since the first case was reported on 20 May 2015.

While the occurrence of such a large outbreak outside the Middle East is a new development, there is currently no indication that the virus is behaving differently than elsewhere. Like previous outbreaks, the outbreak in the Republic of Korea is associated with transmission in the health care setting and among close family contacts; so far, there is no evidence of sustained community transmission. Therefore, the overall pattern of infection observed previously remains unchanged.

WHO is guiding the public health response in both the Republic of Korea and the other affected countries, in collaboration with partners. The response includes ensuring proper treatment of patients, preventing further cases and gaining a better understanding of transmission patterns and risk factors in this outbreak. WHO is monitoring the situation and remains vigilant.

MERS-CoV has been reported in the European Region

Since September 2012, 15 laboratory-confirmed cases of MERS-CoV infection, including 7 related deaths, have been reported by eight countries in the WHO European Region (Austria, France, Germany, Greece, Italy, The Netherlands, Turkey and the United Kingdom). Most of the cases were imported and did not result in further spread of the virus: in only three cases was there secondary transmission to family members or contacts. The last case was reported in Germany in March 2015.

The risk that another traveller infected with MERS-CoV will enter the European Region remains. Every day, there are many travellers between Europe and the countries that are currently reporting MERS-CoV cases. Experience has shown, however, that European countries are prepared to deal with imported cases of MERS-CoV, including laboratory capacity to detect MERS-CoV, treatment facilities equipped with isolation wards and arrangements for contact tracing. In particular, consistent application of adequate measures to prevent infection and other public health measures has obviated or mitigated transmission.

WHO recommends that European countries be highly vigilant

WHO encourages all countries in the European Region to be vigilant, strengthen their surveillance for acute respiratory infections, especially among travellers or migrant workers returning from the Middle East, and carefully review any unusual patterns. The WHO Regional Office for Europe stands ready to support countries in assessing their capacity to respond to MERS-CoV and to provide technical assistance to scale up preparedness.  

Adherence to infection prevention and control measures in health care settings

It is difficult to identify patients with MERS-CoV early, because the symptoms are similar to those of other respiratory infections. In order to mitigate the risk for transmission to people who might be exposed to MERS-CoV (health care workers treating a MERS-CoV-infected patient), strict adherence to infection prevention and control measures is vital.
Health care workers caring for patients with probable or confirmed MERS-CoV infection should use precautions against contact and droplets—medical mask, eye protection, gown and gloves—in addition to standard precautions.

Public health advice to travellers

Travellers to and from currently affected countries should receive public health advice on MERS-CoV.

Travellers to affected countries, particularly people with pre-existing medical conditions and the elderly, should avoid close contact with animals, especially camels, and follow strict hygiene measures, such as regular hand-washing.
People should avoid drinking raw camel milk or camel urine or eating meat that has not been adequately cooked.
Travellers back to Europe should report any acute respiratory illness to medical staff during 2 weeks (incubation period) after their return and inform the staff about their recent travel. Eliciting a travel history from patients with severe respiratory disease is crucial for detecting cases.
WHO does not recommend the application of any travel or trade restrictions or entry screening related to MERS-CoV.

Abu Dhabi, UAE – January 27, 2016: The Health Authority – Abu Dhabi (HAAD), the regulatory body of the healthcare sector in the Emirate of Abu Dhabi, announced today that a 73-year-old has passed away after contracting MERS Coronavirus (MERS-CoV).

Details of the case

A 66-year-old, male, Qatari national developed symptoms on 18 February while in Saudi Arabia, where he had stayed for approximately 2 months. On 19 February, the patient sought health care in a hospital in El-Hassa Region, Saudi Arabia, where he was treated symptomatically and discharged. The patient developed additional symptoms on 20 February. On the same day, as his condition deteriorated, the patient was transferred by ambulance to a hospital in Doha, Qatar. The patient, who was a heavy smoker and had comorbidities, tested positive for MERS-CoV on 21 February. He passed away on 7 March.

The patient owned a camel barn in Saudi Arabia and frequently visited it. He had a history of frequent contact with the camels and of consumption of their raw milk. He had no history of contact with the other known risk factors in the 14 days prior to the onset of symptoms.

Ministry of Public Health (MOPH) has announced that a new Middle East Respiratory Syndrome Corona Virus (MERS-CoV) case has been confirmed for a 40-years old resident works as a camel worker in Qatar, marking the second MERS-CoV case to be confirmed in the country in 2016.

The case, who neither was in contact with a suspected case nor he has been abroad during the last two weeks, is free from chronic diseases that usually cause immunity suppression. He was admitted to Hamad General Hospital with general symptoms where the routine investigations tested positive for MERS-CoV despite he did not exhibit any respiratory related symptoms. The patient currently in stable condition and receives the necessary medical care in the isolation ward according to the national protocol of the infection prevention and control.

Once the case has been confirmed, the rapid response team of MOPH has carried out extensive search to list all potential contacts with the purpose to check for their possible consistence with the standard case definition of the suspected cases based on the WHO guidelines. All traced contacts will be monitored over a period of two weeks, while those who develop suspected symptoms will then be subjected to confirmatory laboratory investigation.

While research efforts continue on a global and local level to determine the modes of transmission of MERS-CoV infection, the Ministry of Public Health advises citizens and residents who suffer from chronic diseases to avoid direct contact with camels and to wash hands with soap and water thoroughly. Also MOPH recommends to implement respiratory hygiene and cough etiquette with the need to boil camel milk before drinking.
